Simplify almost all disconnect() calls

It means less code and lesser error probability. See
TalkableProxyModel::removeFilter() for example - there was a typo
in the second disconnect().

model: Use Qt::DirectConnection in models connections

Models must inform about changes immediately. Really. Also
sthAboutToBeSth() signals must be emitted/taken care of before
something actually changes, not after.
